We may change and republish the Terms and conditions from time to time. It is up to the user to regularly review these Terms and conditions and note any changes. Continued use of the site after a change has been made, indicates the user’s acceptance of those changes.

The company acts as an independent medical tourism facilitator and agent of selected medical/dental care providers. It is acknowledged by you that Crystal M Ltd, is a third-party facilitator and does not provide medical, dental or legal advice, nor is a travel agent. Our aim is to provide our clients with access to quality, world-class medical and/or dental services at affordable prices and it is expressly agreed that no liability whatsoever will attach to us arising out of the performance or non-performance of these.

If you register with our website, submit any material to our website or use any of our website services, you too agree to these terms and conditions. Our website uses cookies; by using our website or agreeing to these terms and conditions, you consent to our use of cookies in accordance with the terms of our privacy and cookies policy.

SERVICE PROVIDER

1.1 The Service Provider, Crystal M Ltd with Unique Identification Code (UIC) 204914625, a limited liability company, is set up and exists under the laws of the United Kingdom, with its registered office Ocean Air House Suite 200, 750-760 High Road Leytonstone, London, E11 3AW

1.2 Crystal M Ltd is not a healthcare entity and does not provide any type of medical and/or healthcare services.

1.3 Crystal M Ltd is an external entity managing the provision of medical and dental services which means it processes sensitive personal data relating to a user’s health that is provided voluntarily. Crystal M Ltd specialises in facilitating dental and medical treatment in Türkiye and Switzerland according to the highest quality standards combined with additional arrangements for foreign patients.

1.4 The dental/medical treatment is provided by selected dental/medical clinics and hospitals that are responsible for the consultation, treatment plan, dental treatment and warranty. Crystal M Ltd through their website www.crystalm.uk provides information regarding the possibilities of treatments and prices for dental and medical care at some of the best dental and medical clinics in Türkiye and Switzerland. The content of this website is provided for your general information only and does not constitute an offer or solicitation of business and may not be treated as such until confirmed by Crystal M Ltd.

1.5 Our role is to act as a third-party facilitator between the medical and dental care providers and yourself. We will always endeavour to work in your best interests as part of this process. The quote that we will supply will include the treatment that you and the provider of health care have agreed upon. All procedures should be thoroughly understood and if there is any aspect that you do not understand please seek clarification

The herein listed terms and conditions include the most important provisions concerning all services offered by the Service Provider. Please read them carefully as becoming our Client/Patient indicates acceptance of our terms and conditions.

DEFINITIONS AND INTERPRETATION

In these terms and conditions, the following definitions shall have the following meaning:

Account – a User’s profile run by the Site Operator, which constitutes an element of the site, unavailable publicly, being a collection of resources where the user’s data are collected, enabling storing data, and vital for providing “make an appointment”, “ask a question”, ” services, secured by a password known only by the user. Via an Account, a user can take advantage of the site’s functions and manage their data and materials sent to the Service Provider. A user is allowed to have only one Account.

www.crystalm.uk Website/Website/Site – specialised internet portal available at the following URL addresses: www.crystalm.uk owned by the Service Provider indicated in point 1 of these terms and conditions. The Website serves as a way to deliver to users materials, information or current price lists in electronic form pertaining to dental/medical services, accommodation, travel and transfers, and spa and wellness services provided within the territory of Türkiye and Switzerland. The Website enables, in particular:

a) obtaining information about available services;
 

b) obtaining information about the prices of services;
 

c) reading and adding opinions;
 

d) making an appointment;
 

e) providing assistance in arranging travel and accommodation;
 

f) providing assistance in dental and/or medical treatment arrangements made between us and users;
 

g) providing assistance in transport between arranged points: health care providers, the airport and the place of the User’s stay in Türkiye and Switzerland.
 

h) providing customer service in English throughout the duration of the user’s stay in Türkiye and Switzerland.

User – a User who benefits from the Services available on the Site in a way that is not related to business or professional activities.

Contact Form – an electronic document enabling communication between the Service Provider and Users, the filling of which is necessary for the enablement of Services.

Contract – a contract for the provision of services described in the terms of service, entered into for an indefinite period between the user and the service provider to the extent and under the conditions specified in these terms and conditions, including the following services:

Information Services – a free service provided electronically, offered through the site; allowing the users to familiarise themselves with offers as well as information on the available dental, medical treatments and examinations, accommodation, tourism, spa and wellness services in Türkiye and Switzerland. Information services do not require any input by the user, who can use it anonymously, regardless of age.

Make an Appointment Service – a free service provided electronically or via telephone, involving making appointments for dental, medical, cosmetic, travel and/or accommodation services; requiring creating an account by the user, entering personal data, and the user having full legal capacity.

Health Services – activity aimed at prevention, maintaining, saving, restoring or improving users’ health and other medical activity resulting from the treatment process or separate legislation for providing them.

Infoline – dedicated phone line/s enabling registration and contact between a user and the service provider’s employee in order to take advantage of available services. Conversations conducted via the infoline may be recorded and stored by the Service Provider in compliance with the law.

Opinion – a user's assessment of a service published on the Site. At the user’s request, an Opinion can be published without providing the user’s name and surname; the only identifier of the user will be a name or screen name indicated by the user. An Opinion is published after verification by the site operator, who verifies if the user used any services and ensures the Opinion does not have contents of an offensive, vulgar, obscene, pornographic, racist or advertising nature.

Online Chat – online communication between a user and a service provider’s employee (consultant) involving sending text messages via the chat facility on the site. The discussion is meant to help in the understanding of the provided services and is used to register users.

Price Lists – information on approximate prices of services offered to users from the service provider. Information contained on the site, including price lists, does not constitute an offer within the meaning of the provisions of the Türkiye and Switzerland law.

Service Provided Electronically – a service provided without the simultaneous presence of the parties (at a distance), through the transmission of data on an individual request of a recipient, transmitted and received using devices for electronic processing, including digital compression and data storage, which is entirely sent, received or transmitted with the help of a telecommunications network.

Site Operator - Crystal M Ltd.

Site User – anyone can use the site as an information service; anyone who has a device with a web browser and access to the internet and who goes to one of the following domains: www.crystalm.uk./co.uk/uk. Other services can only be accessed by a User who is an adult having full legal capacity, capable of giving consent for health care services and incurred liabilities.

Registration – the process of creating an account for a user, necessary for the delivery of services, consisting of sending the user’s personal data necessary for the distribution of services, done by filling out the contact form available on the site and providing data to the Infoline consultant via telephone or online (online chat).

Terms of Service – these terms of service, whose acceptance is required for the realisation of services.

CONSULTATIONS

Initial consultations may be free subject to a particular promotion of the service provider or the charge may be deducted from your total bill at the end. If you start treatment within 4 weeks of drawing the treatment plan, we may refund you the cost of the consultation once the treatment is done. The price of the initial assessment depends on actual offers and you will be quoted the exact amount prior to finalising your appointment. During your consultation, a panoramic X-ray (OPG) will be required to assess the state of your teeth and mouth.

Because dental health circumstances can change rapidly, quotations are valid for 30 days and only if there has not been a change in the dental health conditions which would require additional dental work than that quoted for. Within a few days after your initial assessment you will receive a personalised treatment plan (or plans) from your dentist where you will find adequate information on the treatment options available to you together with their consequences, costs and possible risks. Once receiving the treatment plan (plans), on your request, you will be able to discuss with your dentist, over the phone, (or via other communication channels) the options available in order to select a course of treatment that is medically sound, aesthetically pleasing and financially affordable to you.

Dentistry - In most cases, details of the work you require along with panoramic x-rays of your mouth will be sufficient for your dentists to create your treatment plan. However, this may need to be altered, or tailored, when you arrive in Türkiye and Switzerland and are examined in person.

YOUR TREATMENT

Treatment Planning - Once you have discussed and agreed with your treatment plan, we will provide you with a written estimate for the cost of your treatment If this plan changes due to radiographic or clinical findings, we will inform you and discuss this with you accordingly. Treatment plans are valid for 30 days from the date the treatment was prescribed. If there are any questions on your treatment plan, please do not hesitate to ask for clarification, prior to commencing treatment.

Consent Forms - Certain treatments require the completion of a written consent form. This is to ensure that you understand what was explained including the treatment, aftercare and any risk to you thoroughly, before any of these treatments are carried out. It also allows you to make an informed decision before agreeing to treatment.

Before the performance of the dental treatment:
 

1. Before every procedure, every patient has to sign a written consent form.
 

2. The treatment is always carried out in, Türkiye or Switzerland.
 

3. The medical treatment in, Türkiye or Switzerland is always preceded by a clinical examination, which is performed during the first visit to the dental clinic. The aim of the medical check-up is to specifically verify: information and data provided by the patient, medical conditions of the patient, prospects of performing dental treatment and confirming or stating total treatment costs.
 

4. If, for any reason, the patient is not present on time and or is not in a state where the dental check-up can be performed, the appointment will be cancelled, and the service provider will not be able to give compensation. In certain cases, the service provider may need to rearrange the appointment. In that case, the service provider will notify the patient as soon as the need for rescheduling arises, and offer the patient alternative times.
 

5. The service provider reminds you that the proposed duration of treatment is approximate, based on the provided information, and that the treatment duration may change after clinical examination due to various medical conditions. Service providers cannot accept any responsibility if a longer stay is required. If your stay is shorter, your dentist will do their best to get the required dental work done in the available time.
 

7. The service provider notifies you, that the proposed cost of the medical service is estimated on the basis of information and/or X-ray provided by you. The cost of the medical service may differ due to the necessity for performing additional services.
 

8. The number of required visits depends on the patient’s oral and medical conditions and may vary depending on the individual case.
 

9. Once the booking is complete, we will send you a confirmation email with your booking details.
 

Treatment Options - It is our aim that you receive adequate information on the treatment options available to you, together with their consequences, costs and possible risks. You may want to discuss the options available with your dentist, in order to select a course of treatment that is medically sound, aesthetically pleasing and financially affordable to you. Please make sure you understand the implications of your treatment and the alternatives before agreeing to any treatment.
 

Dental treatment package prices are valid only in conjunction with a package in its entirety and do not apply to any single component thereof. Crystal M Ltd is not able to provide you with a breakdown of the cost for each individual component of your purchase. No refund will be given for unused or partially used package components. All promotions are subject to availability and cannot be combined with any other Crystal M Ltd offers or rebates.
 

Adjustments made to package pricing will not be made after payment has been applied. After payment has been made, no subsequent rate reductions will be applied retroactively. Crystal M Ltd discount cards can be used for all treatments except for yearly check-ups, consultations and scaling and polishing.
 

Course of Treatment - In some cases, it may only become apparent after the commencement of your treatment abroad that the course of treatment discussed during your initial assessment has to be modified. Sometimes this happens as old crowns are being removed and loose teeth pulled. At other times it may happen at the very onset when specialists are consulted. The implications of such findings will be explained, and a modified treatment plan will be agreed. In some cases, the modified treatment plan may involve an extra trip abroad.

During this process, you may at any time decide not to continue with the treatment. In this instance, you will only have to pay for treatment and laboratory work which has been provided. Using temporary solutions (dentures/crowns/bridges) in the long term could harm your remaining teeth or implants. Temporary dentures mean improper loading on natural teeth destroying supporting structures (gum and bone) causing increased mobility and possibly the failure of those. Plastic temporary crowns/bridges are not sealing the same way as the actual ones. Due to the gap between teeth and these types of crowns food is getting stuck causing decay and gum disease in the long term. Ineffectively fixed temporary dentures and debris-collecting crowns/bridges also mean permanent irritation on implants and can cause their failure. In such cases if the patient does not follow the treatment timeline and refuses to show up on the upcoming visit within 4 weeks from the designated time given in the treatment plan Crystal M Ltd has the right to withdraw any discount given previously.

 

The service provider has the absolute right to decline or discontinue services at any time without any obligation for compensation if you are not medically or mentally fit for the recommended treatment or if your payment obligations are not met. The service provider also has the absolute right to decline further service if:
 

1. the patient is found to have withheld information or provided false/incomplete details regarding his or her medical history and
 

2. if for professional, medical or ethical reasons, the dental specialists do not deem it advisable to proceed.
 

In some cases, your appointment time and/or the place of treatment and/or the treating specialist may need to change. In this event, we will contact you to advise you of those changes at the first available opportunity. Your dentist may at any time without notifying you make any changes to the treatment which are necessary to comply with any applicable safety or other statutory requirements, or which do not materially affect the nature or quality of the Services.
 

Travel - Travel, accommodation, meals and other auxiliary expenses of treatment abroad will in all cases be borne by you, regardless of whether those expenses are incurred in respect of consultation, treatment, check-ups, guarantee or any other reason.
 

Please always allow plenty of time for flights as we may not be able to reschedule your appointments. If you do not allow sufficient time for your stay abroad and laboratory work is delayed, you may need to make an additional trip.
 

Purchase of Services - When you make a request on our website, we shall email you a booking confirmation email. Once you have checked out and you have received your booking confirmation email it will not be easy to make any changes to your booking so, please make sure that everything is correct before placing your order.

We continually update our website. From time to time we may run special promotions (which may not be available on our website) or we may offer special discounts online that are not available otherwise.

Fees - Crystal M Ltd does not offer credit or financing for medical or dental treatment and we require fees to be settled at the appointment where treatment is provided. Fees for certain treatments like dental implants and clear braces are taken in staged payments at each visit. We will discuss and plan a schedule of payment with you.

PRICING

Guide Price - The guide price, often given prior to any consultation or diagnosis, is provided to give you an idea of the cost range of a procedure, treatment or price plan, and will be accompanied by what the guide price may include. The guide price is not a confirmed or guaranteed total price for any service. All the fees applied for any form of dental treatment at the dental clinics are based on the materials used, laboratory costs incurred (if applicable), and the clinical time spent. The fees are, however, demonstrated as treatments prescribed on the treatment plan for your information.

Quote Information - The quote you receive may also list the costs of other services such as accommodation or local assistance, and will be broken down so these separate items are clear. We do not provide all-inclusive prices or packages. The total quoted price when given may include, (a) surgery, treatment or procedure, (b) transfers from the airport in the city of your choice to the accommodation or medical care provider on arrival and the reverse transfer for departure, (c) consultation with your dentist/doctor, (d) any pre-operative tests advised, (e) accommodation out or at the medical care provider’s premises, as advised (f) hotel stay, (if arranged through us), for the duration of your stay.

Not Included - Any tests considered necessary during pre-assessment or diagnosis for some treatments, or as a result of a pre-existing condition, or your exposure or vulnerability to any condition will be at additional cost. These may not appear on your quote or invoice prior to travel as they might be decided on later, while you are at the clinic. Payment for any additional services required or chosen in this way is to be paid directly at the clinic in question.

Hotel or Other Accommodation Bills - You are liable for any other costs that you incur such as meals (not included as part of our negotiation on your behalf), use of telephone, laundry, etc. at the hotel, or stay at accommodation not booked through us.
 

Flights - you are responsible (unless otherwise agreed) for booking and managing your own flights. We will advise you on carriers and times where possible, but booking and paying for flights will not form part of our agreement with you.
 

Payment Terms - Upon an agreement being made for a booking, the following terms apply:
 

For logistic services packages, full payment of the invoice prior to travel. Any extra or unforeseen services are to be paid additionally upon agreement.
 

For dentistry, you have to arrange your payments at the clinic. Any extra or unforeseen services are to be paid at the clinic.
 

Payment Options - You can pay using various forms of payment including American Express, Visa, Visa Delta, Visa Electron, Maestro, MasterCard, Solo, and PayPal.

Further Information for Overseas Customers - When you pay with your card for your purchase payment will be taken in the currency of your account. The actual price charged to overseas customers will be subject to the exchange rate applied by the customer’s credit or debit card company.

Credit Card Validation - All credit/debit cardholders are subject to validation checks and authorisation by the card issuer. If the issuer of your payment card refuses to or does not for any reason authorise payment then you will be notified of this immediately at the check-out stage.
 

Remedial Care - In the event you need to return for remedial care service provider will afford reasonable support in its capacity as a “facilitator. “In the event of a dispute between you and the clinic where you have received treatment, this will be conducted under the laws of the Republic of Türkiye or the confederation of Switzerland. As a facilitator, we will continue to act between you and the treatment centre and assist with you with the process wherever possible.
 

Medical Information and Questionnaire - In some cases, you may be required by the Service Provider to complete a medical questionnaire or provide medical information prior to consultations or treatments. You agree that your medical and screening questionnaires and assessment questions will be shared with your examining and treatment centre. The service provider is not liable if your questionnaire has not been completed thoroughly or honestly or if any information is withheld or incorrect. We will not disclose your data, records or information without your consent and will only share it with those concerned with your treatment.
 

Delays - We do not accept any responsibility for any delay to flights or other transport methods even those booked by us. When flight delays occur, refreshments and meals will normally be the responsibility of the airline. Flight timings and days of operation are subject to change. The service provider will not be held responsible for any such delays or changes to these or related services and you agree to indemnify Crystal M Ltd against such eventualities. However, we will endeavour to assist you in ensuring any transport issues are rectified as quickly and comfortably as possible.
 

Insurance - It is recommended that you have travel insurance that will cover delays, loss of baggage, personal effects, etc., as well as sickness and ill–health. We also recommend you take out Medical Travel Insurance and/or Medical Complications Insurance for the treatment you wish to have. You must decide if the provisions add to your level of comfort and if you wish to subscribe to the policy, but we highly recommend covering yourself in this way whenever travelling abroad for medical treatment.
 

Cancellation - In the event of a missed appointment or an appointment cancelled with less than 24 hours' notice, you may be charged the full fee for the consultation. For appointments longer than 1 hour, at least 72 hours notice is required. Cancellations for scheduled dental treatment are only accepted in writing (e-mails or post sent directly to Crystal M Ltd) and must be received no later than 72 hours (3 working days) in advance. In the case of cancellation for a patient’s first visit, there is no cancellation fee. However, due to the restrictive nature of the timing of treatments, a cancellation charge of £50 may be charged for the cancellation of a subsequent visit.
 

Should you be late for your appointment your dentist can refuse to see you that day and can offer you a different date. We will try to call all patients 48 hours prior to appointment to confirm their attendance. Once confirmed, you cannot change the time and date of visit. If, for any reason, you are not in a state for the consultation to be performed, your appointment may be cancelled, and we will not be able to give you compensation.
 

Guarantee - Guarantees, if offered by your dental clinic, depend on the surgery and/or procedure and will be part of your agreement with the treatment centre where you will be treated. It is not the responsibility of the service provider to provide guarantees, and the company cannot be held liable for such provision.
 

Guarantees whenever provided are subject to the following conditions being met:
 

  • The patient has fully paid for the treatment and does not owe the practice any money for the treatment received.
     

  • The Veneers, crowns, bridges and dentures have not been damaged as a result of an accident, trauma or excessive grinding.
     

  • The Patient has followed all post-treatment maintenance recommendations made by our dentists.
     

Liability - Your treatment is subject to an agreement between you and the dental/ medical clinic undertaking the treatment. Staff from Crystal M Ltd do not hold any medical qualifications and are neither responsible nor liable for any medical or dental procedures or for the work of the surgeon, dentist, or any of the staff, engaged at your clinic or hospital. You agree to indemnify Crystal M Ltd against the incidence of medical or dental negligence or malpractice. These aspects will invariably be governed by the terms and conditions of your treatment contract with the health provider.

We do not accept any responsibility in the unlikely event that the dental/medical clinic that made your treatment plan feels that you are not fit enough to undergo surgery and have to return without your surgery or procedure taking place. You will be entitled to a refund of any unspent balance of your treatment cost after deductions on account of tests, consultations, etc.

Your dentist/ doctor will give you aftercare instructions and advice. Should you not follow these and there are repercussions arising from your own lack of care, your dental clinic may not accept responsibility and other parts of this agreement may be void.

The service provider will act as the patient’s representative. It is acknowledged by you that Crystal M Ltd does not provide dental or medical services and it is expressly agreed that no liability whatsoever will attach to it arising out of the performance or non-performance of these trading terms or the treatment plan or any modifications to it.

The service provider’s staff may assist you with the booking of appointments, hotels, apartments, transfers, and sightseeing but you will be solely responsible for settling all and any costs arising directly with the service provider/operator. Under no circumstances whatsoever will the service provider be liable for any losses howsoever arising from the performance or non-performance of any service provider/operator (flight delays, luggage loss. etc.)

The service provider can help with hotels, apartments, and sightseeing arrangements but all customers are responsible for any cancellation. All fees or charges imposed by other travel suppliers, such as airlines, hotels or tour agents, will be paid by the customer. Please note that for some hotels or apartments once your reservation is confirmed, your reservation may be wholly non-refundable.

While we have tried to provide accurate and timely information, there may be technical or factual inaccuracies and typographical errors, for which we apologise and we accept no liability for any inaccuracies or omissions on our website to the extent permitted by law. We reserve the right to make changes and corrections at any time without notice.

Please remember that medical and dental information, treatment information packs, guides or website descriptions provided by Crystal M Ltd must be considered as an instructive service only. The information sent through e-mail should not be relied upon as a medical consultation. This mechanism is not designed to replace a physician’s independent judgement about the appropriateness or risks of a procedure for a given patient. We will do our best to provide you with information that will help you make your own healthcare decisions.

Sensitive Information - Crystal M Ltd may collect personal information that is ‘sensitive information’, and may relate to health or medical details (as medical reports). This sensitive information will be used or disclosed only for the purpose for which it was given and forwarded directly to the party it is intended for or as required by law. Where personal health information such as medical records or reports are required to be transferred to a third party, this will be in conjunction with the completion of the consent form (release of medical information form).

In order to perform our services, you will be required to disclose certain personal information, including your personal data, travel plans and health records to Crystal M Ltd or any agent appointed by us to receive such information. No liability will arise in respect of any losses suffered by you where the information disclosed is found to have been incorrect or incomplete. It is your sole responsibility to ensure that the information provided is accurate and to immediately notify Crystal M Ltd, or any agent appointed by us to receive such information, of any changes to it.

Crystal M Ltd will always respect your privacy and any personal communication between you and us or any associated company. The information that Crystal M Ltd collects from you is only that required by Crystal M Ltd or any associated company to provide you with the information, products or services you requested. Crystal M Ltd does not trade, rent or sell client details to third parties. Should Crystal M Ltd's intentions change, it will be done only with your authorisation. Crystal M Ltd will always comply with any data protection legislation currently in force.

Personal Details - It is very important that you give a full medical history and details of any medication you take. Should these change in any way, it is very important for you to tell us and your Dentist. It is the patient’s responsibility to inform us and the treatment clinic of any changes in either personal details and/or their medical history.

Complaints - At Crystal M Ltd we always take complaints about any aspects of our services very seriously. We aim to ensure that every patient has only the very best experience at all times. Complaints can be made in writing. Complaints should be made to the ‘Crystal M Ltd Complaints Manager’ and should be clear so that they can be dealt with efficiently.

Every complaint will receive immediate written/email acknowledgement and the Management will strive to resolve the complaint within a quick, reasonable period of time (usually about 2 weeks).

No Tolerance/Abuse Policy - We operate a zero-tolerance policy to abuse to our staff, loud/disorderly/drunken behaviour, and persistent missing and late cancellations of appointments (after multiple warnings). In these situations, the service provider reserves the right to refuse treatment and communication.

Circumstances Beyond Our Control - Except where we say differently elsewhere in these conditions, we cannot pay any compensation, expenses, losses or any other amount or otherwise accept responsibility if we have to change your booking after departure or if we or your dentist cannot provide you with your procedure as agreed, or you suffer any loss or damage of any description as a result of circumstances beyond our control.

When we refer to circumstances beyond our control, we mean any event that we or the provider in question could not foresee or avoid even after taking all reasonable care. Such circumstances will usually include war, terrorist activity, civil unrest, industrial dispute and bad weather (actual or threatened).

Disclosure - You hereby agree with these terms and conditions as set out here. These terms are a contract between us and you may wish to keep a copy for future reference.
